    Original and photocopy of the student's PSA/NSO birth certificate.

  • Report Card

    Original and photocopy of the student's report card from the previous school.

  • Good Moral Certificate

    Certificate of good moral character from the previous school.

  • ID Photos

    Four (4) recent 2x2 ID photos with white background.

  • Medical Certificate

    Medical certificate indicating the student is fit to study.

Transferees

  • Birth Certificate

    Original and photocopy of the student's PSA/NSO birth certificate.
